Registration, Overage and Transfer Forms Section
A player trying out for a BC Hockey Competitive, Rep and Zone teams outside of Chase Minor Hockey (your Home Association) requires specific forms to be signed by a representative of Chase MHA before the tryouts begin. Please contact our Registrar or President if you have specific questions regarding participating in out of Association tryouts or transfers.
The attached document describes to proper procedure for trying out for a team in a division or team other than a Recreational Chase MHA team

Please choose the appropriate form from the link below. Please note that each form must be completed PRIOR to any player going on the ice with an Association other than their Home Association and forms must also be pre- approved by OMAHA
Transfer With Parent form is a required document for all Minor Hockey players who are registering with a different Home Association than the one that they were previously registered *Updated 06-28-23*
Overage Player Request form must be completed to apply for an overage player exemption under specific circumstances. *Updated 06-28-23*
Permission to tryout Form must be completed to tryout for a team in U15 or U18 Female A divisions *Updated 06-28-23*
Permission to tryout Form must be completed to tryout for a team in U13 Tier 1 division *Updated 06-28-23*
Permission to tryout Form must be completed to tryout for a team in U15 (Bantam)AA (Tl) or U18 (Midget) AA (Tl) *Updated 06-28-23*
Residential Waivers Section
Residential Waiver Recreational-must be completed in order to move from CMHA to play in another Association if CMHA does not offer a Recreational team in the players birth division
Residential Waiver Carded- must be completed in order to move from CMHA to play in another Association if CMHA does not offer a tiered team in U13 or Female A divisions
Residential Waiver Other- This form can be used to request a transfer in an adjacent Association under certain named or exceptional circumstances
